These are the Appendices (1-8) to the Conflicts of Interest Consultation Paper published on 10 October 2011 by the Conflicts of Interest Working Party of the Professional Regulation Executive Committee.  Appendix 8 contains a hard copy of the questionnaire accompanying the Consultation Paper.  However, we would prefer if you filled out the questionnaire online at: http://www.surveymonkey.com/s/actuaries_conflict_of_interest.  Responses to the Consultation Paper are due by 10 December 2011.
