Students are required to produce a form of photographic identification when appearing for each examination. The photographic identification is to be placed on the examination desks so that it may be checked during the examination by an invigilator. The following are acceptable as identification:
Students who do not currently hold one of these forms of identification, will be expected to obtain one before the date of the exam.
This form should be used when a candidate is unable to produce satisfactory proof of identification when sitting for an examination. If you fail to produce such proof, your scripts will be marked but the results will not be released until the Profession is satisfied of your identity.
On completion of the form Part A must be retained by the student. Parts B and C should be sent to the Registrar, Napier House, 4 Worcester Street, Oxford, OX1 2AW.
You are required to complete this form up to 3 weeks before the results are published. However you are strongly advised that all matters concerning proof of identity should be resolved before this time.
The document, with all sections completed, dated and signed should provide sufficient proof of identity to the Board of Examiners who are responsible for the examinations.
The student must consider the following instructions:
Students will only be allowed one session where photographic identification is not produced, and they must complete a verification of identity form.
Please note that if you fail to produce some photographic identification on more then one occasion then the Board of Examiners will consider whether or not to mark your script.
